For a Personal Application, you need to fill out box numbers 1 (Date), 2 (Name), 7a (Home Street Address), 7b (City), 7c (Province), 7d(Postal Code), 7e (Home phone number) and sign in Box 16. You do NOT have to fill anything else out.

For a Business Application, you will need to fill out Box number 1 (Date), 2 (Personal Name), 9 (Business Name), 10a (Business Street Address), 10b (Business City), 10c (Business Province), 10d (Business Postal Code), 10e (Business Phone Number), 11 (Type of Busines), and Sign in Box 16. You do NOT have to fill anything else out.

Once your Personal or Business Application is completed, you may fax it along with (2) Photo ID's to (518)312-4318 or email it to info@ccmailmanagement.com

Acceptable identification includes: valid driver's license or state non-driver's identification card; armed forces, government, university, or recognized corporate identification card; passport, alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or a home or vehicle insurance policy. A photocopy of your identification will be retained in a customer file.

After the application with Photo Id has been received and payment has been made, we will send your US Address by email within 12 hours. You also may call to get your address. **Please note that the address is Not valid until payment, application, and id has been submitted. **
